The Health Department will be here @ Eastside High School @ 1:00 on April 8th to administer vaccinations for Meningococcal, Tdap & HPV Vaccines. The 2nd dose of Meningococcal (MenACWY) is required for up-coming 12 graders. The Health Department is requesting to fill out on-line using: linospring24
If you can not fill the consent form out online then please fill out the paper copy of the consent form and return to the school nurse as soon as possible.
