The LENOWISCO Health District is pleased to be able to offer a school-based adolescent vaccine program to assist parents in meeting the requirements for students who are rising 7th graders or rising 12th graders to receive school required immunizations. We hope you will take advantage of this opportunity to comply with this school enrollment requirement.
School-based clinics will be conducted during the month of April 2024. These will be held at Alternative Education Center on April 9, 2024. The consent forms must be returned to your child’s school no later than March 27, 2024, with a copy of the front and back of your child’s insurance card.
